iCATHI Home Care Systerm

 

 

The CARI system manages health and social aspects to provide a support mechanism for those with complex medication needs or where memory impairment is a concern.  These are scheduled on a website which monitors responses, creating Alerts when specified actions are not completed.

 

iCATHI is the Patient aspect of this system—a simple screen with big buttons to confirm each item and it ‘disappears’ but updates the web record.  Alerts can be sent to carers, friends or relatives.  The prompts can be maintained through controlled permissions for example medical staff to edit medication and relatives can change social items.

Transplant Recovery

 

iCATHI will prompt the patient with all medication, clinic and hospital or district nurse appointments with the ability to monitor drug intake and visit outcomes with alerts generated if critical items are missed.  These alerts can be based on different time delays and be sent to different recipients (such as Family, friends, Care Team, Consultant etc.) depending on the issue involved.

 

Where blood test results need to be checked for medication changes the Care Team can be prompted to do so - and then any changes can be automatically reflected in the patients prompts.  The ability to measure drug intake and blood test results enables better support for post operative patients and can enable discharges to be brought forward with confidence with savings of up to £5,000 a week a more.

Complex & Critical Care

 

Where the medication and care regime comprises a number of services from several Providers and specialist medical support there can be a bewildering number of things going on.  CATHI can cope with all these issues, providing:

   Reminders for the Patient

   Alerts for Carers and family or friends

   Track Provider attendance & Reconcile costs

Elderly Care At Home

 

Many older people would prefer to stay in their own home but memory loss is a serious problem.  The iCATHI package for these people will cover medical and social care aspects but also things like birthdays.

 

Here we see family, friends and volunteer agencies getting more involved in support and text or email alerts could be sent to them too!

 

 

 

Transplant Recovery:  Regimes extend hospital recovery time.  Real time monitoring of the patient post-discharge improves outcomes while reassuring patients, families and friends.

 

Complex & Critical Care: Here monitoring is not immediately critical but where there are home care and nurse visits or medication is involved iCATHI monitors actions to alert when necessary.

 

Care At Home:  Where memory loss is a factor or missed medication can lead to emergency admissions, reminders of medication, visitors, clinic or hospital appointments can reduce mistakes.  It also supports people at home better for longer, reducing the need for expensive residential care.

Needs Assessment is available foruse by teams in other locations or organisations to create a Patient - centric view of both medical and social needs.

 

The Commissioning body is then able to select the most appropriate Provider, with contract / cost information to enable an approved budget to be established.

 

The iCATHI system can then be used both to keep the Patient informed and to track attendance and link to invoice validation.

 Enabling Patients to recover or be supported at home is an essential element of changing the way we deliver better care at lower cost.

 

Bed blocking is a major cost and capacity issue - savings can easily pay for iCATHI support.

 

Beyond the ability of Care Professionals using iCATHI for selected Patients there is scope for the system to be used for multiple Patients in a Care Setting.

 

It is also viable on a personal use basis with friends or family maintaining the system.
